T-LPN (Job ID OOJ - 7204)

Responsibilities:

• Provide direct patient care, ensuring comfort and safety.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient conditions, vital signs, and report any changes to the RN/physician.
• Assist with wound care, IV therapy (as permitted), and other clinical procedures.
• Support patient admissions, transfers, and discharges.
• Maintain accurate and timely documentation of patient care.
• Collaborate with healthcare teams to implement individualized care plans.
• Educate patients and families on treatment plans and health management.
• Follow infection control and safety protocols.
